Creating two profile where each have Invert Health Color set differently seems to cause the issue.
If I change the profile the effect does not get updated on health bar, however I can see that in profile they have different settings.
In one occasion, when it did update, half of health bar was inverted and the other half not! but as soon as I healed the target it fixed it
