Serve index.html for directory paths #73

Addresses https://github.com/rgrove/rawgit/issues/72

Not too familiar with Nock fixtures, so I just kinda stole from an existing one, leaving a lot of the (likely incorrect) headers.

Tested and works great. I'm happy to rework with your feedback. :+1:

Mostly looks good. Will merge as soon as https://github.com/rgrove/rawgit/pull/73#discussion_r32272004 is addressed. Thanks!
