a already uploaded file appear in mail notifications of other files #267

What steps will reproduce the problem?
1. upload some files with a client account
2. upload some other files
3. in mail notification we can have the first list of files again added to the 
second

What is the expected output? What do you see instead?
Expected only the new file uploaded, we have some other extra files.

What version of the product are you using? On what operating system?

I am using r412 but am also getting this and other issues.
When I upload a file for a client the client receives a notification about the 
file and  other files he/she is able to upload (but not all).

Also another client also received notifications but they were not the files 
that were uploaded and I had not allocated my uploaded files to that client.

Now we have been having trouble sending emails but that is now rectified.
I had done 9 sets of uploads each giving a valid email notification.

Now I can understand how the first may have precipitated the notifications for 
the other user but not all (unless the notification status has not been updated 
correctly)

I have had a look at the database.
For the files uploaded and correctly notified the files are still showing a 
staus of not notified. Attached is a print out of the relevant tables.

The time stamps of the fils show between 15:00 and 17:00 today 6 Aug 2013
