Password verification was not completed Passwords do not match

What steps will reproduce the problem?
1. Add new Client name and password, and then submit
2. Get- Password verification was not completed Passwords do not match
3. Re-enter password again , and then it works.

What is the expected output? What do you see instead?
Should not have to re-enter password on a second submission. It does not seem 
to know the first submission passwords were correct.

What version of the product are you using? On what operating system?
Latest r412 - but it started doing this on the previous version r405?

Please provide any additional information below.
Using Firefox.
Using a three letter, three number password. Example password: bob123

This should be fixed on the next version, only a single password field is used.
