Insert Operation in Contact object #75

Hi,
I am trying to insert data into Contact object in Salesforce but it doesn't get 
inserted, instead I am getting error in log file saying "Required fields are 
missing: [LastName]". I have already mapped Last Name as I know that it is a 
required field. Then, why does it say that required fileds are missing?
Please let me know,

The mapping file is case sensitive - did you check that?
