Docs feedback - add an option to include an email address #145

Previously, the docs feedback option captured the email address for the person who left feedback. That enabled the team to reach out for clarification or to provide links to relevant docs.

With the new feedback system, we do not have a way to respond to feedback. Add an option for people to leave their email address when they provide feedback.
