121 qual status value missing of format 3 subformat 3 in v203 version #123

Added missing values to Format 3, subformat 3 table and cleaned up table formatting.

Thanks Bill. Can you update the revision history and update the version number by incrementing the final digit please? Or are these kinds of formatting fixes not being tracked in other specs?

Correct, other specs don't track formatting fixes or fixes to issues caused by the Asciidoc conversion.
