Project Improvement to Align with LFX Best Practices #37

The following changes have been made for the template and as an enabler for derived projects:

Created Code of Conduct file that points to RISC-V Code of Conduct
Created a GOVERNANCE file that points to the Documentation SIG
Created a MAINTAINERS file that list Bill T as current maintainer
Created CHANGELOG file that starts by listing our last release (4.0.0)
Updated the README to include references to the new files.
