Miscellaneous cleanup across the specification #51

This patch includes the following changes which are mostly semantic.

  1. Uniform usage of must instead of shall
  2. Typo fixes
  3. Remove references from interrupt table
  4. Remove stray white spaces
  5. Align boot process section/subsection names between base & server extension
