Added video upload feature #96

Added feature that enables doctors to upload their own videos and patients are able to see those videos in their assessment; Removed option to select videos from Youtube

oh also, can you clean up the package.json? I imagine there are packages we don't need any more, like the youtube uploader. if you see any other things that it's clear we don't need, can you remove them too? if you're not sure, just go ahead and leave them in to be safe.
