Add PostgreSQL output type #4

Create new output for PostreSQL databases.  Should operate similarly to MS
SQl and MySQL

Hi Charles,

Here is attached a postgresql output that I wrote, if you want add it. It will 
take care of fields with dots and uppercases since I doubled quoted the fields.

Hi Stephane, thank you for the work :)  I will definitely get this in the next 
release - postgresql has needed some love.
