What is the vendor's name? Jotform

What is the vendor's pricing site? e.g. https://slack.com/pricing

What is the base pricing? Use the lowest tier that looks sane for a small business customer, not free or personal tiers. $24/mo (Billed annually)

What is the minimum pricing for SSO support? $594/mo (billed annually) Enterprise Plan + $207.50/mo (billed annually) SSO Add-on = $801.50/mo (billed annually)

Does this pricing info come from a quote or other non-public source? Pricing came from a quote. I can provide the quote privately for verification if needed.

Are there any caveats we should list in the footnotes? Base plans are single-user only whereas enterprise plans are multi-user.
