Should have the following fields

Expiration date Checkbox for whether or not it's received The received date. If it's received you can edit the received date. else the received date is cleared out

Consolidate the caregiver form instance add to a general form instance add (or maybe only caregiver form instance add). Then make the form/template/views work with editing.
