individual osd stats (throughput, iops, etc) #11

Should be possible to plug in metrics regarding throughtput, iops, etc into graphite, the same way we do for pools.

Couldn't figure out a way to get this info. The built-in plugins to check filesystems are used instead, we can monitor individual partitions with them.

The only data available is commit/apply latency and kb_used, which has been added to the pg plugin (#3).
