Creating a new authoring name appears to fail but actually succeeds

We're running IR+ 2.0.4 on Linux.

In Firefox 3.6.12 for Windows, I'm encountering the following problem:

1. Click on the My Account button.
2. Click on the Authoring Names tab.
3. Click on the New Name button.
4. Enter first and last name and initials, and select Authoritative Name.
5. Click on Submit.
6. The new name does not appear in the list; you have to refresh the page to 
see it.  This leads to duplicate names.

This appears to happen only when adding the first authoring name, and (perhaps) 
only if it's the authoritative name.

No error appears in our log file (ir_plus_web.log).

Was able to reproduce looks like a javascript error

This has been fixed but a new issue should be started to separate the mixture 
between using admin and user code - as changes in one area affects the other
