We currently miss underspecified mutations like p.?, c.?, p.0, p.0?

changed grammar to account for p.0? and p.0 (see http://www.hgvs.org/mutnomen/examplesAA.html#sub) and for c.0 (see http://www.hgvs.org/mutnomen/standards.html)

p.? and c.? are not following any nomenclature
