[Feature] Add boolean option valid social security number (AMKA) #214

User Story As a HRC health lead I want to gather data on if people have made a visit with a valid or invalid social security number So that I can report on this.

Describe the solution you'd like A boolean option in Visit, for valid AMKA.

This should then also show in the dashboard (issue #175)
