User Story As a HRC health lead I want to know how many unique individuals that received a vaccination had valid and how many invalid social security number So that I can report on it using the dashboard

Describe the solution you'd like Add a checklist field in Vaccination entity, mandatory and max count =1

Did this and closing issue. Now needs to be added to dashboard, see issue #222
