Change order of Terms of Contact and simplify form #88

Some changes to facilitate user access: After clicking in "Entrar na sala de espera de triagem" show: 1) Form with CEP, name and age (omit CPF field)

Then, if there is a Barracão for the CEP, show: 2) Terms of service 3) Contact number/email (both optional)

And omit medical info form, we can check this during the video call.

We had thought of making these changes to facilitate user access since there is in average 80 new visits per week to the website but few people go ahead and fill all the form. I think we still can go forward and make these changes, maybe just keeping the CPF field for now.
