Contribute create page div rearrangement #1079

The contribute.html page has some divs that needs to be re-arranged / close properly. Check out L#148 and L#153.

Remove closing div in L#148. Remove closing div in L#153. Add closing div for div class="col-sm-10" and div class="form-group" . Check where should the div class=container L#93 should be closed.
