doc: Revise plugin installation instructions #71

Consolidate the instructions to eliminate duplication, and fix the location of the administration page (it's under Settings now).

There was no mention of this style checking. I will fix it, and also add a note to that effect in the Contributing section.

I made a few more changes (e.g., I found that composer test had 2 requirements which are now documented). But as far as the Codacy checking, I'm at a loss. I reviewed all the "issues" and they all look like false positives. FWIW, Codacy also finds a ton of failures on the rest of the codebase.
