Two posts with menu_order 0 #30

In our wordpress install we had two posts that would randomly switch places. I echoed out $post->menu_order and both were set to 0. Dragging the posts in the plugin fixed this, but I'm wondering how this could have happened in the first place? Recently upgraded to 4.7.1.
