Add Data Format section to README #34

This PR tries to improve the README in general and it also clarifies what have been source of confusion in https://github.com/rootstrap/active-storage-base64/issues/25 and https://github.com/rootstrap/active-storage-base64/issues/28 , the format of the base64 data.
