Initial one will simply be via a message header in the plumber call, unless you've got any better suggestions?

"Official" statement from Jeff Allen via this SO question suggests that it's not directly possible in plumber. It has to be done with nginx, for OAUTH2 via nginx-openid-connect. There are some useful examples here.
