Fixes affiliation of HL. #118

Somehow this got messed up, not sure when that happened. Also, making it so that the supercript of the last author is not oddly out of order with the alphabet.

The resubmission may not need to get fixed immediately, but we should make sure we catch this on the galley proofs.
