Add classcodes object for infection/dislocation according to NH #69

Note that "bara om kombinerat med ICD-kod ovan" in reality means that those codes could be ignored, since conditions are already recognized based on ICD-10 codes alone.

Note also that "bara om kombinerat med tidigare höftprotes" is not really relevant here since we will only use this classcodfes with patients who have hip arthroplasty.
