A bit more text, re-organization, spell check #73

@karinorman Can you take a quick look over this and merge when ready?

I added a short example of direct db access, since I think that's actually pretty simple and somewhat compelling. I also moved the table of providers to the beginning, and a few textual edits here and there. (And I added a closing comment at the end of the discussion about the general approach).

Can you also do a once-over pass on the final formatting? In particular, we should check that the float tables are showing up in reasonable spots, that the tables are correctly referenced from the text (many aren't actually being referenced at all by table number :-/) and that the captions for the table are reasonably complete (some are a bit too short, it's not clear if the table is showing the output of the package functions / processes, vs just being a table of some example data).

Other than that I think we're in good shape, but maybe just give it a once through for readability and typos.

Yes! I thought I had gotten all the tables lined up and referenced, but it sounds like with the hard number coding some things got messed up. I will double check!
