I've seen this once or twice, and it just happened again. Very rarely, a user creates a question and then edits it while it is still pending moderation. The question shows up in the moderation queue twice, and when I delete the first copy (since it looks like a duplicate in the moderation queue, and we get those a lot), it also deletes the second copy of the message, and while it looks like it's still in the database, I haven't managed to figure out how to undelete when this happens.
