Refactor PR Instructions GHA scripts and add instruction for previewing CONTRIBUTING.md

What changes did you make?

• Moved all formatting of instructions to create-instruction.js and added URI Encoding
• Add template for instruction regarding CONTRIBUTING.md
• Provide preview URL for CONTRIBUTING.md based on contributor's fork

Why did you make the changes (we will use this info to test)?

• URI Encoding was added because the backtick character caused a problem
• The existing pr instruction is not accurate for previewing CONTRIBUTING.md, because CONTRIBUTING.md cannot be previewed locally
• By moving instruction formatting to create-instruction.js we can add code to format instructions based on the files changed in the PR
