Create Leader / Add Project form should be clean after viewing exisiting Leader or Project #150

Steps to reproduce and a minimal demo of the problem

  1. View any Leader.
  2. Try to add new one.
  3. Observe the form.

Current behavior Form fields are pre-populated from the viewed Leader's details.

Expected/desired behavior Form fields should not be pre-populated from the viewed Leader's details.

Other information The same behavior can be observe for a Project.
