Disable middle click paste option #384

There are any number of ways one could accidentally middle-click (particularly 
while scrolling or on a crowded desk), and since a wayward paste can execute 
large swaths of unintended code, it would be nice for those of us who don't use 
the feature to be able to eliminate that risk.

I wanted to disable this too.  I used to have an extremely stiff mousewheel 
which would cause frequent middle-clicks when scrolling.  It was usually only 
annoying but pasting newlines into mintty could be very destructive.

I added a new radio control on the mouse page which will let you select between 
paste, extend (the old action if you had right-click paste), and nothing.

A picture is included for reference.

I have also increased the height of the config window as nobody wants 
overlapping controls.  This was surprisingly hard to find in the source.
