Test that dedication doesn't goof up toc location in PDF bookmarks #32

I added tests related to this in f37bebad8bfdfaa7f5600d181bb1df146680506c and a fix in the previous commit, but I don't know how to programmatically investigate the PDF bookmarks to ensure that Dedication shows up as page ii and Table of Contents as page iii.

Fixed in commit ed3696f7a8a09742b425ccb5fcec66ab4d3d4089.
