Remove references to Pthread for Windows Build #86

Windows Visual studio project build requires pthreadVC2d.dll in same directory 
as the build.  While pthread allows multi-threading support, references to 
pthreads were agreed to be removed due to the lack of integration in a prior 
July meeting.  Thus simplifying some of the Windows build process.

A systematic removal of pthread references is required.

Note: some complication in IDtable
