Slow startup of Viessmann and Glowmarkt binding under 4.0.0 #48

The following WARN log message is generated:

Channel types or config descriptions for thing 'xxx' are missing in the respective registry for more than 120s. In case it does not happen immediately after an upgrade, it should be fixed in the binding.

This appears to be due to OpenHAB now expecting channel types to be ready before the binding has interrogated them from the server.
