Add more icons to the menu #3443

The main menu could do with a few more icons, the Unit Tests and Refactor submenus have plenty but other submenus are a little barren. It would be good if we could also add one or two to the root menu - perhaps Refresh and Settings?

Refresh and Settings, definitely (we already have those icons)

Code Explorer needs one as well, ...and everything under the Tools submenu actually.

Indent Procedure could use one too, but only because it's the first "indent" command (would be too crowded with an icon for every indenter command) - I figure we could come up with an icon made with the "method" purple block and something that represents "indentation", hopefully SharpDevelop icons exist for that.
