Specify custom Polaris development domains #23

Describe the solution you'd like

We only support .dev.my.rubrik-lab.com for Polaris development mode. Since there can be multiple dev accounts that can be used we need to allow the end user to specify their own. This should for sure be in the default settings but may be interesting to add this to the form itself as well.

There is a bug in the current implementation that needs to be corrected.
