Refactor: update soft-deprecated form helpers #5576

Description

Was wondering whether I could update your one instance of form_tag and 25 instances of form_for, which are soft-deprecated, to form_with?

Happy to close issue right away (if I have permission to do so) if deemed unnecessary.
