allow staff to modify sign-in workflow without writing code #96

Requested by GrowHaus. The proposed method is to embed a google form rather use a rails form, so this would be a big change.

After discussing it with the GrowHaus (and a bit of soul-searching on my part), we ended up replacing the entire app with a google form. There is a bit of formula wizardry in the responses spreadsheet to calculate the duration of each activity, but otherwise everything is changeable/configurable via google apps GUIs, which they consider to be a huge benefit.

Ben, it's a good decision - the problems of funding a server and ongoing code maintenance needed to be addressed upfront. Thank you for pushing this over the finish line!
