Addresses Issue #84

@ajohnson052 is this just adding the date-picker not actually add it to the appointment creation? Is that the distinction btw this and Issue #84 ?

@pollygee no, this does add the date picker to appointment creation. Should I use fixes #84? I was hesitant to automatically close the issue in case I had missed something
