add configuration parameters overriding environment variable

add following settings.

okazukiplantuml.javaHome: overriding JAVA_HOME
okazukiplantuml.plantUmlJar: overriding PLANTUML_JAR
okazukiplantuml.graphvizDot: overriding GRAPHVIZ_DOT overriding PLANTUML_HOME setting does not exist because it is not needed when PLANTUML_JAR is set

this would be cool, windows is annying in setting env vars
