Is there a magical way we can make an index? ASCIIDOC supports indexes with macros:
indexterm:[<primary>,<secondary>,<tertiary>]
but this doesn't support ranges (e.g., pp. 5-10), sees (e.g., Rails, see Ruby on Rails), or see alsos (see also Ruby on Rails).
A stand-alone index could also be made, simply referring to anchor tags on each page.
