The index is amazing! Here are the outstanding issues (in order of priority). We can live without 2-5, but 1 I think is necessary.

1) Eliminate duplicate page listings, i.e.

Body (Box2D element) 220, 224, 224, 225, 225, 225, 226, 227, 245

should be:

Body (Box2D element) 220, 224, 225, 225, 226, 227, 245

2) Add ability to have "sortas", i.e.

((("A New Kind of Science (Wolfram)" sortas="New Kind of Science (Wolfram)")))

3) Be able to escape quotes somehow, i.e.

((("This listing has \"quotes in \" in it)))

4) Support page ranges, so

Body (Box2D element) 220, 224, 225, 225, 226, 227, 245

becomes

Body (Box2D element) 220, 224-227, 245

Also, we need a way to add italics and quotation marks to the titles of books and articles.
