Add documentation for procedures around issue tracking #17

This is intended to establish the procedures for managing Council issues as brought up in #10 and discussed at https://rust-lang.zulipchat.com/#narrow/stream/392734-council/topic/Backlog.20issue.20tracking. This is intended as a starting point, and I expect things to change over time to adjust to how we actually end up using things.

I'm going to merge this, because I don't want to risk this staying in PR form for too long. We can keep iterating as we encounter things during real use.
