Add `place_combined_point` table #339

Follow up to #337.

I'm second guessing this one. I feel like there is likely enough duplication between the point and polygon tables to make this just a big mess. Taking off the version, not sure it's worth adding in. I might get around to trying to back that gut feeling up with data, if someone else makes the case to add it I'd be open to that.
