Make accessible PDF version

Already put in effort to make the book easily readable, but dyslexics would benefit from additional things (typeface, line and letter spacing, type size etc). Make an accessible version.

Dirk Schlimm doesn't like the meta variable font; and it certainly doesn't go with the Go Sans. Maybe just boldface for the metavariables?

With the availability of an accessible HTML version this is no longer a priority/good enough for what it needs to do (ie basically a large print edition)
