Simple grammatical fixes mostly resolving issues resolving singular v plural

ln 3 - rather than in "local/controlled environment," it should be "a local/controlled environment" or "local/controlled environments"

ln 33 - specified plural or singular. The incidents are also assumed to have been bad and thus is redundant. It offers more coverage to say "We are not responsible for any such incidents" than specify bad incidents anyhow.

ln 43 - "The following" is more correct than simply "Following"
