Currently Mage Death is handled two different ways, one on a phase change basis the other on an Event trigger () during the Action Phases, which triggers each time either Life or Damage counters are incremented or decremented. The problem is most players will not advance to the next phase to trigger mage death and as a result the game never ends and a winner is not declared.
