Document CSV format in README #17

The CSV format expected by the app ((address,amount)) should be documented in the README.

While writing https://github.com/sablier-labs/v2-interfaces/discussions/674, I realized that it would be best to post some example CSV files in a GitHub repo. Maybe here:
