Final changes to CS 4 #28

I made some final changes to CS 4 before submission.

The code is now using the plot method to plot the hazards.
I've updated the Intro and Discussion to highlight time-varying covariates.
I removed some of the more problematic plots (e.g. hazard ratio, absolute risk with time-varying exposure)

@sahirbhatnagar Just so you know: All of these files with conflicts are by-products of compiling analysis/paper/paper.Rmd, so it should be straightforward to merge this pull-request. Let me know if you want me to do it.
