GFormIt does not correctly implement the authorization lifecycle #107

What steps will reproduce the problem?
1. Install the GFormIt add-on
2. Go to the "Add-ons" menu for a Google Doc
3. Select "GFormIt for Docs...;" you only see "Help"

What is the expected output? What do you see instead? There should be a 
"Generate Form" and an "About GFormIt" menu items as well. See authed 
(expected) and unauthed (unexpected) images. The issue is that the 
authorization lifecycle, as described at 
https://developers.google.com/apps-script/add-ons/lifecycle#opening , was not 
implemented correctly.
